Removal of a hard plastic bottleneck causing penile strangulation with holmium laser
A 44-year-old male presented to our emergency department with a chief complaint of a hard plastic bottleneck strangulating around the base of his penis for 3 hours. The patient inserted his penis into a bath lotion bottle for masturbation, of which the bottleneck was approximately 1.8 centimeters in diameter. Due to penis swelling, congestion and pain, the patient found that the penis could not be pulled out after 2 hours. He tried to remove it completely by himself but failed. As the patient was concerned about penile necrosis, he sought emergency care at our hospital.

Whole Exome Sequencing Identifies a Rare CFTR Mutation in Brothers With Anomalies of the Vas Deferens: a case study
Congenital bilateral absence of the vas deferens (CBAVD) occurs in almost all men with Cystic Fibrosis. Prevailing theories on this pathophysiology relate to pathogenic mutations in the Cystic Fibrosis Transmembrane Regulator (CFTR) gene leading to agenesis or obliteration of vas deferens in utero. In this study, we present a case of two brothers with congenital anomalies of the vas deferens who were found to have carry a rare, heterozygous CFTR variant p.r347h without pulmonary or gastrointestinal signs or symptoms of CF.

Within State Variability of Antimicrobial Susceptibility: Missouri as an Archetype to Assess Guidelines for Antimicrobial Prophylaxis for Transurethral Procedures
To evaluate variability among hospitals in susceptibility of common uropathogens to antimicrobial agents frequently used in transurethral procedures in order to examine whether state-based guidelines might be more appropriate than national prophylactic guidelines.

Staphylococcus epidermidis as a uropathogen in children
Historically, Staphylococcus epidermidis has been considered a contaminant when grown on urine cultures. However, a growing body of literature suggests that S. epidermidis can cause urinary tract infections (UTIs) in children with anatomic variants or a history of instrumentation. In this case report, we describe a previously healthy child who presented with symptoms of UTI and urine cultures grew this uropathogen. The patient was screened for anatomic abnormalities and none were found; nonetheless, appropriate treatment should be initiated even if no underlying pathology is found.

Unexpected pediatric uretero-inguinal hernia: case-report and literature review
Uretero-inguinal hernia (UIH) is a rare condition in children. We present our experience with an unexpected UIH in a male infant with associated anomalies and a literature review of pediatric cases.A full-term male newborn was referred to our hospital for microcolon and right hydroureteronephrosis. Voiding cystourethrography showed grade-IV right vesicoureteral reflux and a dilated, convoluted, ureter protruding in the right inguinal canal, suggesting a right UIH, which was uneventfully repaired at two months of life.

Efficacy, Efficiency, and Safety of En-bloc vs. Three-lobe Enucleation of the Prostate: A Propensity Score-Matched Analysis
To assess efficacy, efficiency and safety in holmium laser enucleation of the prostate (HoLEP), we directly compared intraoperative performance, postoperative outcomes, and safety in the original three-lobe enucleation technique with the more recent en-bloc method. As HoLEP is widely accepted as a size-independent method for surgical treatment of patients with lower urinary tract symptoms (LUTS) secondary to benign prostatic obstruction (BPO), detailed understanding of its benefits is mandatory.

Bilateral adrenal and testicular tumours; when a simple diagnosis becomes complicated
A 78-year-old man with a BMI 28.7, a moon face and a medical history of diabetes mellitus, hypothyroidism and undefined adrenal insufficiency, has been in urological follow up for both prostate cancer (PCa) and a non-muscle invasive high-grade urothelial cell carcinoma of the bladder (UCC). PSA remained low and stable after External Beam Radiation Therapy (EBRT) and adjuvant androgen deprivation therapy (ADT). Cystoscopy revealed no recurrence of malignancy after transurethral resection of bladder tumour and instillation of BCG.

Testicular Compartment Syndrome after Trauma Managed with a Tunica Vaginalis Flap
Testicular compartment syndrome (TCS) requires timely diagnosis and intervention but may be challenging. We present a case discussing the presentation and management of testicular compartment syndrome following testicular trauma in an 11-year-old male. The patient presented 24 hours after testicular trauma from a kick with testicular enlargement and sharp pain. Ultrasound showed markedly decreased blood flow and a reactive hydrocele. Testis-sparing intervention included emergent tunica albuginea incision, debridement, and tunica vaginalis flap.
