Selection of Finalists, Winner and Prizes. (a) Each entry will be prescreened to ensure that it qualifies and otherwise complies with the Official Rules. (b) One Grand Prize Winner (“Winner”) will be selected at random between November 1, 2009 and November 7, 2009.(c) Any act that unfairly, deceptively or otherwise improperly affects the selection process is a material violation of the Official Rules.(d) ONLY ONE GRAND PRIZE WILL BE AWARDED TO ONE OVERALL WINNER. The Grand Prize Winner will receive up to $500 towards the cost of one (1) airline ticket. Ticket much be purchased between September 10, 2009 and December 31, 2009 for flights within the United States that occur between September 10, 2009 and June 30, 2010, and receipt of purchase must be provided to UrologyMatch via email (collectingsystem at urologymatch.com) by January 31, 2010.(e) At the sole discretion of UrologyMatch, the first name of the Winner may be used as part of any informational, public relations, advertising, promotional, marketing or media campaign by UrologyMatch whether online or otherwise. No compensation will be provided to Winner in the event an entry is selected for such use. (f) Other conditions and restrictions apply. Prizes are non-transferable. No substitutions or redemptions allowed. Total value of prizes: $500. (g) By accepting the prize, the Winner releases and discharges UrologyMatch, its employees, officers, directors, representatives and agents from any liability or damage due, in whole or in part, to the award, acceptance, possession, use or misuse of any prize or from participation in this Contest.
