Match Horror Stories

Has anyone heard of the stories of candidates getting 12-15 interviews and still not matching?  I have, but I dont understand why.  Can someone please explain?

Possible explanations

This happens a couple of times a year. Here are my suspected reasons from talking to people from years past.

1. Applicant looks great on paper but was a terrible interviewer and rubbed every program the wrong way.
2. Interviewed at > 12 places but ranked much fewer places
3. Interviewed at only top programs that are much more selective, and even though they look great on paper and interviewed well, they never impressed one program enough to rank them highly
4. Location bias: applicant grew up in one state, went to undergrad and medical school there and program outside that state feel that applicant would not adjust outside their home state
5. Unlucky

1.) douchbags, weird, akward, etc.
2.) They were cocky and didn't rank enough programs
3.) They only ranked top programs (along the lines of stated reason #2)

There are probably other reasons but these are ones that immediately come to mind.
